In short
The best man is the groom's chief supporter; the maid of honour (or matron of honour, if she is married) is the bride's. Traditionally the best man keeps the rings safe, organises the bachelor party and gives a speech at the reception. The maid of honour leads the bridal shower and bachelorette party, helps the bride on the day and may also give a toast. In the US, both often sign the marriage licence as official witnesses (Wikipedia: Best man; Maid of honor).

In Germany the role is called Trauzeuge or Trauzeugin ("marriage witness"). Since 1 July 1998, witnesses are optional at a German civil ceremony – couples may name one or two if they wish (§ 1312 BGB, in German). The Catholic Church still requires two witnesses.
Example
Ben asks his sister Clara to be his "best woman". She keeps the rings, plans the bachelor party and gives a speech that has the whole room laughing – then sets up a few wedding games.
What to watch out for
- Agree early on which tasks are actually wanted.
- Church or civil rules may differ – check with your officiant.
- On the day itself, your best man or maid of honour needs support, not extra jobs.
